INTAKE APPOINTMENT INFORMATION

The intake appointment will take approximately 3 hours. 
What to Bring:

  • Valid Photo ID (drivers license, state ID, military ID, passport).
  • Proof of your social security number (card, pay stub).
  • We accept cash, money order, or cashiers check only.  Sorry, no personal checks at the time of intake (personal checks are accepted after the first week of treatment). 
  •  If you owe any back fees from previous treatment with us, we are willing to work out a payment plan with you.

You will begin by meeting with a counselor to complete paperwork and an assessment.  Then, a nurse will check your vital signs and complete lab work. Finally, you will meet with a doctor who will assess your withdrawal symptoms and determine an appropriate starting dose of methadone or buprenorphine.  You will be required to submit a urine sample for testing prior to meeting with the doctor. All intakes are breathalyzed prior to being dosed and if there is a positive reading the completion of your intake will be delayed.
